Delisting of shares

Hi all,

can any one provide me the procedure of delisting of shares at Calcutta Stock Exchange with all relevant formats of the documents to be submitted.

Prohibition of insider trading reg., 2015

Dear Sir,
As per SEBI (PIT) Regulation, 2015 under Regu. 7(1) required to disclosed the holding by promoter, director and kmp to the Company. The said disclosure in Form A is required to inform to the Stock Exchange by the Company or not??
