my exam form pdf has my photo and signature on it , however there is a blank space for the sign and membership no of ca on 2nd page bottom right , do i have to post he same after attesting

As I Joined in Article ship on 28/11/2014, I Completed my Group I & I applied for Grp-2 (Ipce) for May 2017 Attempt, Do I need to Scan any Certificate (Document) along with my form while filling the Form.
Any One Please suggest me in this Regard.
