Hi Guys, We are private co., we take service of transportation of our goods (Inward of Raw Material / Outward of Finished Goods) from an individual who owns a truck. He never issue Consignment note. Whether Service tax liability will incur on us under RCM? We are the person liable to freight (i.,e. Service Receiver). Also provision of RCM under GTA will be applicable?

You are not liable for service tax under RCM. You are transporting the goods not through GTA, so provisions of RCM under GTA not applicable.
