the incorporation forms for a new private limited company (INC-7, INC-22 and DIR-12) are marked for user clarification via Addendum form 67 with the remarks:
1. "Photos not pasted in MOA". 2. "Address mismatch with bank statement"
In respect of 1st remark, can any one guide as to how and where to paste the photos in MOA and what is legal provision for than, and
In respect of 2nd remark, can any one guide - do we need to mention the same address in subscriber sheets as mentioned in bank statement (submitted as address proof). In the present case, the address of subscriber mentioned in subscriber sheet is same as mentioned in their DIN application (Voter Card address).

2.Yes. Whatever the address you are mentioning in eform should be supported by vaild documentary proof.
1. Should the address as per bank statement (address proof)match with the address mentioned in subscriber sheet or it should match with address mentioned in E-form. Because, the address mentioned in e-form is automatically picked by system from DIN number. Whereas address as mentioned in bank statement is different.

While fully agreeing with Metha Ji's observation, the possible solution I envisage is:-
If address in bank statement and subscriber sheet matches, e-file DIR-6 attaching bank statement so that address matches on approval of the said e-form.
Then prepare an affidavit stating that DIR -6 is filed, approved and thereby address matches, notarize it, and e-file through form 67.
