03 October 2015 If you need surname, middle name and surname, it can be known from

www.incometaxindia.gov.in --> know your assessing officer --> enter PAN and captcha code --> click submit button.

'Know your assessing officer' is the 5th item in the 2nd row under 'tax service' in the appearing screen.
