14 April 2011 What is the procedure for cancellation of PAN?

and which type of form is to be filled up and submitted?

14 April 2011 request for change in PAN or correction in PAN form 49A is to be used .just tick the option at bottom i.e.surrender of PANs and attach copy of PAN card to be surrendered alongwith ID and residence proof.
