If you know the computation of net wealth, the return is available at http://incometaxindiaefiling.gov.in/ for e-filing or at http://www.incometaxindia.gov.in/Pages/downloads/wealth-tax-return.aspx for other modes of filing.
If you do not know the computation, it is not possible to teach the same in this forum. Please consult a CA or any appropriate professional.
