5) PROOF OF RESIDENCE ADDRESS: (Any two of the following) proprietor A) COPY OF PASSPORT B) COPY OF ELECTION VOTING IDENTITY CARD C) COPY OF DRIVING LICENCE D) COPY OF LATEST PAID ELECTRICITY BILL E) COPY OF LATEST MAINTANCES RECIPTS D) RATION CARD E ) NOC OF FATHAR

7) FOR MUMBAI LOCATION DEMAND DRAFT IN FAVOUR OF BANK OF MAHARASHTRA A/C MVAT (REGISTRATION FEES 5030/-& 25000/-) & IN OTHER LOCATION DEMAND DRAFT IN FAVOUR OF STATE BANK OF INDIA A/C MVAT (demand draft only nationalize bank ).
