Urgent---completion of extended articleship

26 August 2011 I will be completing my extended articleship on 31st August 2011. In this case, i will be required to send form 108 for completion of extended articleship.

But i have a querry regarding what details are to be filled in form 108 under number of days served ---

1) whether to enter details of full 3.5 years of actual days served.
OR
2) whether to write details of working days served only during period of extended articleship.

and one more thing i have served extended articleship for a period of 4 months 19 days so in which colum to write these details?

Please answer this urgently.
