05 September 2013 since it is in hindi, we are not able to answer.

But normally, service tax is payable in two ways, 1) Thro TR-6 Challan in the notified banks by cash or cheque. The bankers will help you to draw the cheque in favour of ....
They will provide you acknowledgement.

2) Thro online -in the respective service tax heads.
