I think even if you are going to serve the additional period under any other principal then the same will be considered as a transfer which may not be allowed by the institute. So first contact the institue whether the same will be allowable or not.
Secondly you can dowlaod the form from think link http://www.icai.org/resource_file/7510form%20107_std.pdf and the same needs to be franked and there is no need for any other form. The form 107 is like form 102 only.
