I have formed my proprietorship firm to provide banking financial services on 01.08.2013 and have applied for registration of Services tax. My query is what all documents are required to be sent along with Application form print out. Also were we have to send these papers.

The following documents are required to be attached with ST-1 form Pan Card Copy, Rental agreement/address Proof Power of attorney to collect and make modification (if application is submitted by representative) Office Address is mentioned on the acknowledgment.
@ CA Umesh Sir, as mentioned in the query it is a new firm and bank account can not be opened without any government dept registration how can i provide cancelled cheque. Please guide on this issue waht to do.
