What is the procedure to be followed for a Single (Decentralised) Registration under Service Tax. Also, what are the documents required for the same?

create a login id on aces. fill st-1, take the print out and sign it with the stamp at bottom also attach : Copy of PAN card of the assessee.
Proof of the address of the premises to be registered/copy of telephone bill. Electricity bill. Rent agreement in the name of the company or in the name of proprietor/ partner/firm in the case of proprietary or document accepted by any of Central/ State Government Department e.g. i.e. Income Tax. Sales Tax. Registrar of Companies DGFT or passport in the case of address.
In case of partnership firm: Copy of Partnership Deed.
In case of companies: copy of Memorandam of Association.
Letter of authority in the name of person who will go and collect ST-2 on your letter head.
A declaration, that will inform about the description of services you providing
