The procedure involves many documents and depends upon the status of would-be-directors etc. For example would-be-directors will start from getting DIN. New PAN and TAN shall be applied.
To convert into Private Limited Company first decide the Authorized Capital, Paid Up Capital, AOA, MOA are to be written - in which the nature of business etc.shall be disclosed.
With the Registrar of Companies all the procedure is ONLINE.
