Salary refers to basic+DA on which 12% PF applicable. BASIC and DA should be minimum 50% of gross wages and it should not be less than minimum wages Act. You can apply online registration portal www.epfindia.com

It depends on company ,company can exclude from PF Deduction. Some companies is consider maximum Rs.15000/- for PF contribution of BASIC and DA.
