IN A/Y 2012-13 HIS TOTAL INCOME FROM COMMISSION FROM INSURANCE COMPANY IS Rs 1,50,000. WHICH I.T.R. WILL BE APPLICABLE FOR HIM ?

You need to file ITR -2 But if you want to claim the Expenses you should go for ITR 4, But be careful Income Tax Notices are very casual for the one filing ITR 4
