Form to be submitted on completion of articleship

03 January 2012 Dear sir,

I have completed articleship traning on 23-12-2011. What is the form to be submitted? what is the due date for submission? To which office should the form be sent? Is there anything else that needs to be sent along with the form?
Kindly clarify

3. no any documents required to be attached with form no 108
