delay along with my employers comments. I hav no Idea wht to reply plz help me

You received Form 112 because you would have done any other course along with CA without the permission of ICAI.
Send following to Regional Office of ICAI:
1) Form 112 duly filled in and signed.
2) Condonation Fees.
3) Reason of not getting permission from ICAI.
4) Your employer's comments on his Letterhead.
