plz let me know, ihad completed my articleship on 15 jan 2012. only 1 copy of form 108 is required to sent to the institute ? nd m from western region so plz also inform me address . is it necessary to fill form 119?

Send the original copy of form 108, and keep the xerox copy for your reference. No need of sending form 119.
