NO, in the case of death of the director no DIR-11 is necessary as DIR-11 is pursuant to section 168(1) which specifying the provisions related with resignation of the director
And As far as DIR-12 is concerned a copy of board resolution and a copy of death certificate

@Dharmendra ji .. See Sir Death Certificate is a mandatory requirement as Evidence of cessation. However, Copy of Board resolution can also be taken as proof of cessation. DIR - 12 is STP form and will approved. however, if it comes in the notice of respective ROC, then he will definitely ask for Death Certificate
