Can any one send me Format of Affedavit for Father Name as passport copy carry only mother name for application of DIN

In the cited case, I am afraid, an affidavit will not serve the purpose.

Sir, But Din cell is accepting the same for approving the DIN. I have done the same for a Geramn National who is a director in a indian private limited based at bangaloare.

Sir,Without father's name in the DIN applicant's passport did you made an affidavit for father's first name, middle name and last name for the said German National.
