WHEN I FILL A MCA FORM AND SIGN IT, LATER IF I HAVE TO MODIFY IT I CANT DO SO. WHEN I CLICK ON MODIFY, DSC ATTACHED WITH THE FORMS ARE NOT REMOVED.

Use a form afresh. Thats it This problem may be because of the system configuration you have. Can't help.
