Please advise:
What should be collected by employer as proof of investment to consider the deduction (u/s. 80CCG) from salary income?

When Demat a/c is opened under 80CCG scheme, employee is required to mention the same & Form A is required to submit to the DP.

You can ask for a statement where iy would have been mentioned that A/c is opened under 80CCG
