firstly u need to write a letter to icai on plain paper mentioning the reason for proposed transfer and need to give sufficient supportings in this case and only when icai permits + ur exisiting chartered signs on form 109 then only u can fill up form 109 and submit to icai else it will be rejected by icai.

on the gov doctors letter head he must mention all the medical grounds and all the supportings like exray,or any reports for test conducted etc which proves that whatever is written in the letter head is correct.

In case of medical basis . 1)a simple letter requesting transfer should be prepared adressing ICAI,dont bother with the format, prepare on your own, recommended to prepare printed and there after sign it so that letter will be seen goood(should specify reason for transfer)
2)your boss recommendation for transfer with the reason specified by you
3)attach government doctor medical certificate to support your reason
