Procedure for completion and termination of articleship . Which form to be filed ? and again procedure for extension of articleship ??

For completion of training, submit form 108 and 119... If there's official extension, thn need to fill form 107.... U can get these forms downloaded from ICAI Site, and filling details r given in the forms itslf...
