16 December 2009 What is the procedure of applying to chief commissioner of Income Tax for the purpose of clause (ii)(b)of the proviso to section 17(2)for claiming exemption in respect of medical facilities.

Is there any specified format of application and what are the other requirements for this. Please provide me all the requirements.
