Form 15H needs to be filled and submitted to the TDS Deductor (Companies). I do not think you have to submit the same to DEMAT (As they are just holding the shares in Digital Forms) and its the companies who are responsible for deducting TDS on dividends if its exceeds Rs. 5000 in a year . TDS Rate is @ 10%

A senior or super senior citizen, who is a resident of India, can submit form 15H to a company or mutual fund house if they are a resident and they do not have any tax liability on their total income for the financial year
