Inaccurate means if any of the details in the return are wrong then penalty may be imposed.
Examples are, mentioning of challan no without actually paying the challan; Mentioning wrong PAN of the deductee just to avoid the 20% TDS; Mentioning any unintentional information with intent to evade tax, etc.
