Staff Salary has to be bifurcated, like doctors'/nurse salary is direct one, but Accounts department salary is indirect one. Doctor's salary also is not perfectly variable, it's semi variable/fixed only

Agree .In hospital Doctors salary directly relates to the serives it can be treated as Direct and Accounts dept is service nature so it can be treated as indirect
