dear sir I want to know if I'm filing return of as asessee who is a doctor in profession his TDs is deducted u/s 194j
i want to ask which return form to use and if ITR 4 is to be used then weather I need to prepare his B/S also
plz reply asap

Yes you should file itr 4 and prepare balance sheet
