If there is a change please provide a format.

06 September 2014 Format of proxy form is prescribed in Form no. MGT.11. A single person can be proxy of not more than 50 shareholders and holding in aggregate not more than 10% of total share capital of the company. A shareholder holding more than 10% of total share capital of the company, can appoint a single person as its proxy, however, such person cannot be appointed as proxy of any other shareholder.
