please fill the PAN form with new name. Do not forget to click/ tick the change in existing PAN and send it across. Ask for the new form. It includes all such instances of rectification/ changes etc.

Go to Any NSDL Office..PAN Utility Service Centre. or Submitt to UTI PAN Facility Centre Nearest to U. Xerox Copy of Certificate n Affidavit With Change Form 49A.
