simply write a letter to the institute, if you can go to the institute make a copy of the same, and deposit the original letter and get the receiving on the other, if you can't go there, sent your letter through speed post or courier. letter to be written with simple language on plain paper and giving the required information, like Your Reg No., attempt. it would be better if you have a copy marksheet from the net, attach that. but not mandatory to attach that.
