The following is the list of documents required for DVAT Registration. There is a requirement of Bank Guarantee of Rs.1,00,000/- along with Form DVAT-12 under VAT/CST Registration.
1 Form DVAT 04 and Form A 2 Self certified copy of the Memorandum of Association and Articles of Association of the Company 3 Self certified copy of Certificate of Incorporation 4 Self certified copy of PAN card of the Company 5 Board Resolution for taking VAT & CST registration in all the state of appointing the 'Authorized Signatory' 6 Details of Directors with following documents: -Address Proof -Proof of identity - Copy of PAN - 4 passport size photographs for each state 7 Form 32 of all directors as on date 8 Details of Authorized Signatory with following documents: -Address Proof -Proof of identity - Copy of PAN -3 passport size photographs 9 Address proof of Principal place and other places of business in the state - Form 18 and copy of bank account passbook attached for principal place - Copy of Lease deed attached for warehouse 10 Bank Account Details - Copy of Bank Pass Book attached 11 List of goods to be purchased against Form C 12 List of principal commodities in which company wishes to deal 13 Bank Guarantee/ Security along with Form DVAT- 12, Rs. 100,000/- each for VAT and CST
