Moa affidavit for signing in vernacular language

24 August 2011 I want MOA Affidavit format for the following MCA query:

MOA MO given in short as per 1A. MOA Affidavit req. for signing in vernaculer language. Form should be resubmitted by 23/09/2011 failing which the eForm shall be treated as invalid and shall not be taken on record.

Please reply as early as possible. Thanks in advance.
