For income tax, you have to give them a copy of the gazette issued by the state government. This shows your old name as well as new. They will change the same. Even Collector (Voter Card, Ration Card and Driving License, Bank etc.) require this Gazette copy. You have to give an application with payment for the booklet in which your name change entry is printed. This gazette entry read with your birth certificate or other documents (SSC, HSC, BCom) is valid proof of your name change.
Passport office in addition to the Gazette Copy wants you to publish advertisement in one national daily and one regional daily newspapers.
