Firm Registration with institute

I want to register my firm wih ICAI. please let me know what is the form for that. also let me know email address and phone numbers mentioned in this form are to be printed on invoices and other papers or i can use another email id for the same.

Are you talking about a CA firm? If yes, please fill form 18 and submit alingwith partnership deed
