15 May 2012 can anyone inform me how to apply to charity commissioner for registration certificate required for application U/s 80G

15 May 2012 apply in Form No. 10G As per Annexure - 29 to the Commissioner of Income Tax

But Form 10G is application for exemption U/s80G. Along with Form 10G we have to submit charity commissioner registration certificate.
And my question is how to apply to charity commissioner for getting registration certificate
