Accounting of income of a doctor

06 September 2012 My Problem is: A lady doctor who is M.D. Gynaecologist gets Rs.2800 per delivery for treating poor caste women after deducting TDS of 13% under Chiranjivi Scheme of Gujarat State. From that per delivery reimbursement of Rs.2800 she has to give following amount to her patients.
If it is normal delivery
Rs.700 for medicine
Rs.250 for fair & tip to person who is with the patient
If it is Sigerian delivery
Rs.2500 for medicine
Rs.900 for Anesthesia
Rs.250 for fair & tip to person who is with the patient
Now, i want to ask what should be the J.E for TDS, Reimbursment from chiranjivi scheme, payment to patient therefrom etc...
I would be highly obliged if you answer me......

06 September 2012 maximum TDS rate is 10% on professional income (194J), hence, in above case TDS rate 13%???
