Income tax refund

One of my client has received an intimation u/s 143(1) and according to that he would get a tax refund of Rs. 16000/-. but he hasn't received any cheque from dept and now refund cheque status is showing Expired : Refund cheque not deposited in bank for 90 days.
Please tell me what procedure to be followed to get it refunded and also plz attach a sample copy of letter to be written to dept.

contact IT Department & consult their officers. Pls also check whether the postal address is correct in IT Dept's record (because the check is not received at the address mentioned in Return).

Write a letter to Local Income tax ward officer mentioned the bank account details and current address details and attached cancelled cheque.
