Dear members, Whlie applying for DIN which is digitally signed by a professional, a copy of the verification by the applicant as per Annexure 1 is being asked as an attachement. Kindly provide me a format of the Verification.
I, ________________________ (name of the applicant) Son of ___________________ (applicant's father name), born on ____________ (date of birth), resident of _______________________ (present address of the applicant) hereby confirm and verify that the particulars given in the form DIN-1 are true and correct and also are in agreement with the documents being attached to the form DIN-1.
I further confirm that
1.The photograph and documents being attached to the form DIN-1 belong to me. I further confirm that all required documents have been duly certified by the respective government authority and are being attached to the form DIN-1 and
2.I am not restrained / disqualified / removed of for being appointed as director of a company under the provisions of companies Act including section 203,274,284 and 388(E) of the said ACT and
3.I have not been declared as proclaimed offender by any economic offence court or judicial magistrate court or high court or any other court and
4.I haven't been already allotted a DIN under section 266B of the companies ACT.

If the DIN 1 form is signed by the applicant himself, then should this declaration be attached to the eform? Can this declaration be in a plain paper? please do clarify.
