This is a vague question. However,there is a form 39 in Karnataka Sales Tax Act-1957,which is the official delivery note.This form was earlier issued by the assessing authority,with some fees,stamp and signature. It was to be accounted in register.Now in its place VAT Form505 is used. This can be obtained on-line.There is also a new form called e-sugam,raised on line, to be used during transit of goods.In KVAT there is no Form 39.....mjk
