Vivad se vishwas

As per Vivad se vishwas scheme 2020, a declarant can file a DECLARATION to an authority in a prescribed manner for a withdrawal of appeal..

My query is whether CBDT has provided any such Specified format/form in which a declarant can file the declaration??

If yes, pls provide the link to get that form.
