Gst reverse charge on service --- return file procedure

Came to know if Reverse charges is applicable of Registered Persons : 

1) To make a self Invoice as per Gst Format and Upload the same to GSTR Return 

Question : 1) Is Self Invoice Serial No. should difference from Regular Sale Invoice ?

                 2) How to upload Reverse Charge Self Invoice to GSTR Return and Which Section under GSTR Return
