Dedution of expenses in case of doctor

A Doctor receive professional fees from hospital. Hospital deducted tds on the same. Can doctor claim mobile exp,  Car petro, Car Dep, & interest on car from the professional fees? & if he fill Form IV for income tax return, is it compulsory to fill balance sheet. can he fill only profit & loss instread of balance sheet & p&l both.
