Dear Sir, I am a Insurance Agent of Max Life Insurance having income of Rs.36000 & tds deducted Rs.3600. whether i should file form 4S or 2?

U R agent so the income arising from Insurance Co. seems to b ur Commission. So u need to File ITR 4. Coz, Commission is treated as business income.
