You have to apply in the demat request slip issued by your depository participant (the agency with which you have your demat account). Attach the share certificates.
The DP will send these documents to the Registrar and Share Transfer agents of the Company. They will verify the certificates and if found alright, make a electronic request to NSDL/CDSL for crediting the shares to your demat account. NSDL/CDSL will do the same and send a confirmation back to the RTA and your DP.
The shares now get credited to your demat account.
