Lost form 'h' certificate of export

20 November 2013 Dear Sir/ Madam,
I am an exporter and I have lost my 'H' form. I have lost it in my ca's office as i have given it in his office for some taxation purposes. Now what is the remedy. I will be very thankful to you.

1.FIR copy

2.copy of news paper publish in this Regards.

3.alonwith request letter for duplicate issue the this crtificate in concerned department.
