There is no specific format, only thing which is require is that following informations are contained in the certificate.
•Name •Status – Individual, company, firm etc •Nationality – in case of individual •Country of Incorporation – for firm, companies •Tax identification number in that country or if there is no such number, any number based on which the person is identified for tax purpose. •Residential Status for the tax purpose •Period for which the certificate is applicable •Address of the applicant for the period above.

Hiren Bhai, Domicile certificate is issued by the HMRC. It is not something you draft. Ask your client to request HMRC to issue the same.

I think Mr. Hiren meant Tax residency certificate required under notification issued on 17.09.2012 to claim DTAA benefit. as explained no specific format, only above information minimum required.
