There is no specific format for it. The waiver letter which I came accross is as follows;
To,
Registrar Of Companies,
Mumbai.
I,....................................., director of ABC pvt ltd, residing at...................................hereby waive the Unsecured Loan of Rs.....................as shown in the statement of accounts of the said company.
